Initial Eligibility

To become eligible for TeamstersCare benefits, you have to work – and your employer must have remitted contributions for – a minimum of 400 total hours over a period of three consecutive months.

For example: If you work 110 hours in March, 200 hours in April and 160 hours in May (a total of 470 hours), and your employer makes contributions on your behalf for these hours worked, you’ll be eligible to enroll in benefits beginning June 1.

Dependent Eligibility

Once your own eligibility begins, your dependents also become eligible for medical, prescription drug, dental, vision, hearing and behavioral health benefits. For the purposes of this plan, eligible dependents include:

  • Your current spouse, or an ex-spouse who meets certain criteria. (See the Active Members Summary Plan Description for full details)
  • Your children, up to the last day of the month the child turns age 26.
  • Your unmarried children who are incapable of self care and dependent on you for support because of a physical or mental disability and meet certain other criteria.
