Log in or sign up and complete the 15 minute Health Assessment and participate in the other wellness activities listed
Why:
Receive a personalized health report and earn up to $200 Wellness Points (gift card dollars)
*Complete a new health assessment as soon as possible (and no later than September 30, 2020). The health assessment must be completed before any additional points can be earned.
